Troponinerhöhung und EKG-Veränderungen bei Schlaganfall und Subarachnoidalblutung

Zusammenfassung

Vaskuläre Erkrankungen sind in den Industrieländern die häufigste Ursache für Tod oder dauerhafte Behinderung. Nicht selten treten aufgrund der hohen Komorbidität Schlaganfall und Herzinfarkt gleichzeitig auf. Deshalb überrascht es nicht, dass Troponinerhöhungen und EKG-Veränderungen nach akuter zerebraler Ischämie relativ häufig zu beobachten sind. Nicht immer handelt es sich bei entsprechenden Veränderungen aber um ein zusätzliches akutes Koronarsyndrom. Vielmehr legen experimentelle und klinische Daten nahe, dass es insbesondere nach Subarachnoidalblutung (SAB), aber auch nach ischämischem Hirninfarkt zu einer neurogen vermittelten myokardialen Schädigung kommen kann, die nicht Ausdruck einer ischämischen Herzerkrankung ist. In diesem Artikel geben wir eine Übersicht über die Häufigkeit und über mögliche Pathomechanismen. In jedem Fall stellen Troponinerhöhungen und EKG-Veränderungen nach zerebraler Ischämie einen relevanten negativen prognostischen Faktor dar. Ein ausführliches kardiologisch-diagnostisches Work-up sollte in jedem Falle erfolgen.
