Virusassoziierte Glomerulonephritis

Zusammenfassung

Die virusassoziierte Glomerulonephritis gehört zum Kreis der para- oder postinfektiösen Autoimmunphänomene. Dabei handelt es sich um immunkomplexvermittelte Prozesse, wobei das virale Antigen im Immunkomplex nachweisbar ist oder vermutet werden kann. Die mit chronischen viralen Prozessen, wie Hepatitis B, Hepatitis C und Humanes Immunodefizienz-Virus (HIV), assoziierten Glomerulonephritiden zeigen im Allgemeinen eine bestimmte virustypische histomorphologische Differenzierung, wie z. B. membranöse Glomerulonephritis bei Hepatitis B, membranoproliferative Glomerulonephritis bei Hepatitis C und fokal segmental sklerosierende Glomerulonephritis bei HIV. Der Zusammenhang zwischen primärer Viruserkrankung und sekundärer Glomerulonephritis ist wichtig, da eine primäre immunsuppressive Therapie der Glomerulonephritis die verursachende virale Infektion verschlechtern kann. Hingegen kann die primäre Minderung der Viruslast durch Immunstimulation oder Virustatika auch den Verlauf der sekundären Glomerulonephritis günstig beeinflussen.
