Ablation von Vorhofflimmern beim älteren Menschen

Zusammenfassung

Vorhofflimmern ist eine im Alter an Häufigkeit zunehmende Arrhythmie. Die ESC-Leitlinien sehen prinzipiell keinen Grund, älteren Menschen eine Vorhofflimmerablation vorzuenthalten. Studiendaten werden in den Leitlinien nicht präsentiert. Ziel des Artikels ist es, die Effektivität und Sicherheit der Vorhofflimmerablation bei Menschen ≥65 Jahre zusammenzufassen und daraus Handlungsempfehlungen abzuleiten: 1) Der Erfolg der Ablation unterscheidet sich nicht zwischen jungen und alten Patienten. 2) In Kohortenstudien zeigte sich ein Trend zu erhöhter Schlaganfallhäufigkeit beim alten Menschen periprozedural. 3) Negative Prädiktoren für Schlaganfall sind Alter >80 Jahre, Herzinsuffizienz, Schlaganfall in der Vorgeschichte und/oder ein CHA2DS2-VASc-Score ≥6.
