Die Arthrodese des Hüftgelenks und ihre Remobilisation

Zusammenfassung

Die Arthrodese des Hüftgelenks als Therapie destruierter und schmerzgeplagter Hüftgelenke wird immer seltener eingesetzt. Dagegen ist die Remobilisation operativ versteifter Hüftgelenke zu einem wichtigen Eingriff in der Orthopädie geworden. Die Indikationen für die operative Hüftarthrodese ergeben sich heute dort, wo zwingende Kontraindikationen gegen die Endoprothetik des Hüftgelenks bestehen oder schwerwiegende muskuläre Defizite die funktionelle Stabilisierung nach einer Totalendoprothesenversorgung nicht erlauben.

Als optimale Stellung eines versteiften Hüftgelenks werden für die Flexion 10–25°, für die Rotationsposition 0–15° Außendrehung, für die Abduktion 0–10° und für die Adduktion 0–6° empfohlen. Zur sofortigen funktionellen Mobilisation des Patienten wird eine stabile Osteosynthese angestrebt. Gangstörungen und vermehrte Belastung der Nachbargelenke an Wirbelsäule, Knie- und kontralateralem Hüftgelenk erzwingen häufig die Remobilisation des Hüftgelenks, die Primärtechnik muss deswegen die funktionsfähige, hüftstabilisierende Muskulatur und ihre Kraftträger erhalten.

Mit dem demographischen Wandel versorgt auch die Chirurgie immer mehr betagte Menschen. Von Entwicklungen wie Fast-Track können auch ältere Menschen profitieren und bei proximaler Humerusfraktur können selbst manche 100-Jährige noch sicher operiert werden.

Patienten, die von Ärztinnen behandelt werden, dürfen offenbar auf bessere Therapieergebnisse hoffen als Patienten von Ärzten. Besonders gilt das offenbar für weibliche Kranke, wie eine Studie zeigt.

Ob bei einer Notfalloperation nach Schenkelhalsfraktur eine Hemiarthroplastik oder eine totale Endoprothese (TEP) eingebaut wird, sollte nicht allein vom Alter der Patientinnen und Patienten abhängen. Auch über 90-Jährige können von der TEP profitieren.
