Sir: We are delighted by the comments of Drs. Schortgen and Vinsonneau concerning our contribution. We are aware that the observed reduction in mortality in the intervention group is dramatic, and we were surprised ourselves. Also, it is true that the control group is an historical one, and thus the analysis was therefore carried out retrospectively. In the control group the mortality rate was indeed very high, but we came to the conclusion of the importance of high-volume isovolemic hemofiltration applied early during the time course of septic shock since no significant differences in other practices were made during the observation periods (control and intervention). Furthermore, it is well known that the Acute Physiology and Chronic Health Evaluation II score is a poor predictor of mortality in patients with acute renal failure [
