Ekzem-Update — klinische Manifestationen, Pathogenese und aktuelle Therapieansätze

Zusammenfassung

Ekzeme betreffen bis zu 10 % der Bevölkerung. Ihre Nomenklatur ist bisher uneinheitlich. Einteilungen sind nach Morphologie, Lokalisation, Auslöser, Kinetik oder Pathophysiologie möglich. Bei der ekzematösen Entzündungsreaktion wandern aktivierte T-Lymphozyten in die Epidermis ein (Epidermotropismus) und es kommt zu pathologischen Veränderungen der epidermalen Keratinozyten mit Spongiose, Akanthose und Parakeratose. Klinisch zeichnen sich Ekzeme durch ein polymorphes Bild aus. Den Hauptanteil stellen die Kontaktekzeme (irritativ-toxische und allergische) sowie das atopische Ekzem (Neurodermitis). Weiterhin häufig sind seborrhoisches Ekzem und mikrobiell-dysregulatives (nummuläres) Ekzem. Entscheidend für die korrekte Diagnose des Ekzemtyps ist die qualifizierte Gesamtbeurteilung der gezielten Anamnese und des klinischen Bilds. Hiernach richtet sich die weitere Diagnostik, um den Ekzemtyp zu sichern, potenzielle Auslöser zu identifizieren und Differenzialdiagnosen abzugrenzen. Da bei den meisten Ekzemen eine allergische Auslösung möglich ist, ist häufig eine qualifizierte allergologische Abklärung erforderlich. Im Management von Ekzemen stehen das Vermeiden/Behandeln der Ursachen und die symptomatische Therapie im Vordergrund. Therapeutisch stehen lokale und systemische Therapieprinzipien mit antientzündlicher, immunsuppressiver, antipruriginöser, hydratisierender, keratolytischer, antiproliferativer, antimikrobieller, antimykotischer oder austrocknend/adstringierender Wirkung zur Auswahl.
