Zusammenfassung

Vierzig Prozent aller Frühgeburten sind die Folge einer aszendierenden Infektion mit konsekutivem Amnioninfektionssyndrom (AIS). Neben infektiösen Erregern wie Mykoplasmen, Ureaplasmen, Gardnerella, Bacteroides, Streptokokken und E. coli spielen möglicherweise auch intrinsische prädisponierende immunologische Faktoren eine Rolle. Zur Prophylaxe kann im Risikokollektiv eine Zervixverschlussoperation sinnvoll sein. Ein Screening auf Vaginosen und deren Behandlung sowie die antibiotische Therapie beim frühen vorzeitigen Blasensprung sind risikomindernd. Das AIS manifestiert sich mit klinischen mütterlichen Symptomen. Bei schwerwiegender klinischer Symptomatik sollte großzügig entbunden werden.
