Sir: We read with great interest the recent contribution by Elliott et al. [
1] evaluating the effect of a sedation-algorithm on the duration of mechanical ventilation in an Australian intensive care unit (ICU). This before-after study showed no decrease in mechanical ventilation [4.8 days (2.1–9.2) in the preintervention phase vs. 5.6 (2.9–10.9) in the postintervention phase;
p = 0.99]. Moreover, length of stay in ICU increased significantly in the postintervention phase [7.1 days (4.1–11.2) vs. 8.2 (4.9–15.6);
