When aminoglycosides are used in critically ill patients, it is crucial that their efficacy is maximised. Aminoglycosides are concentration-dependent antibiotics, and the peak concentration over minimum inhibitory concentration (MIC) is the relevant pharmacokinetics/pharmacodynamics (PK/PD) parameter. Studies have shown that aminoglycosides have their maximal effect at C max/MIC ratio of 8–10. This means that therapeutic drug monitoring (TDM) for efficacy, i.e. monitoring peak levels, may be helpful to guide therapy [1]. …
