Psychiatrische Syndrome bei multipler Sklerose

Zusammenfassung

Psychiatrische Erkrankungen stellen die häufigsten Komorbiditäten bei Patienten mit multipler Sklerose (MS) dar und weisen eine höhere Prävalenz als in der Allgemeinbevölkerung auf. Am häufigsten finden sich depressive Störungen, Angsterkrankungen, bipolare Störungen und Psychosen. Generell sind psychiatrische Komorbiditäten bei MS trotz spezifischer und teilweise validierter diagnostischer Instrumente unterdiagnostiziert und unterbehandelt. So fanden sich lediglich für depressive Störungen bei MS kontrollierte klinische Studien für medikamentöse Therapien. Eine pathophysiologische Differenzierung in Koinzidenzen, Komorbiditäten, medikamentös induzierte oder organisch verursachte Störungen (z. B. im Sinne eines psychiatrischen Schubs) ist aufgrund der multifaktoriellen Genese psychischer Störungen komplex. Die Erfassung und Behandlung psychiatrischer Komorbiditäten bei MS sind klinisch relevant, häufig herausfordernd und bedürfen eines interdisziplinären Ansatzes.
