2. Praktische Schmerztherapie

Zusammenfassung

Nahezu alle Kinder mit lebenslimitierenden Erkrankungen leiden am Lebensende unter Schmerzen. Im klinischen Alltag kommt es oft zu einer eklatanten Fehlversorgung in Form von Unter- oder Überversorgung. In diesem Kapitel werden praktisch umsetzbare Schmerztherapieempfehlungen auf dem Boden wissenschaftlicher Erkenntnisse gegeben. Grundlegendes Wissen über Schmerz im Kontext der Erkrankung, Schmerzempfinden und Schmerzbotschaften sowie Methoden zur Schmerzmessung, auch bei nicht mitteilungsfähigen Kindern, werden ebenso vermittelt wie Fakten zur medikamentösen und nicht medikamentösen Schmerztherapie. Ein besonderer Fokus liegt dabei auf dem WHO Stufenschema sowie der Vorstellung der wichtigsten Analgetika (Nicht-Opioide sowie Opiode) und Adjuvanzien. Ein Unterkapitel über das korrekte Ausstellen von BtM-Rezepten nimmt auch die organisatorisch-rechtlichen Aspekte in den Blick.

Manifestiert sich ein Typ-1-Diabetes bei Kindern, ist das ein Notfall – ebenso wie eine diabetische Ketoazidose. Die Grundsäulen der Therapie bestehen aus Rehydratation, Insulin und Kaliumgabe. Insulin ist das Medikament der Wahl zur Behandlung der Ketoazidose.
