Dear editor: We read with great interest the article by Dr. Cornet and coworkers [
1]. They reported ten consecutive patients with acute respiratory distress syndrome (ARDS) treated with a single dose of 50 mg of sildenafil through the nasogastric tube. The major findings of the study were that sildenafil resulted in attenuation of pulmonary arterial pressures (PAP) and pulmonary vascular resistance (PVR), and to a lesser degree of systemic arterial pressures and resistance. However, they observed a profound increase in shunt fraction, as well as a marked decrease of PaO
2; this observation was not unexpected. …
