Durch eine operative Entfernung primärer Melanome kann die Mehrheit der Patienten mit weniger fortgeschrittenen Tumoren geheilt werden. Trotz der Tatsache, dass die Bedeutung der chirurgischen Therapie des Melanoms bei diesen Patienten immer außer Frage stand, wird über deren Ausdehnung und zudem über die Rolle elektiver Eingriffe in der Behandlung von Melanomen im Stadium I und II immer noch kontrovers diskutiert. Ein besseres Verständnis prognostischer Faktoren und für die Tumorprogression relevanter Mechanismen ebenso wie Ergebnisse mehrerer randomisierter Therapiestudien haben die therapeutischen Prinzipien allerdings zunehmend in Richtung eines sowohl konservativeren als auch selektiveren Vorgehens verschoben. Konzepte in der operativen Therapie von Melanomen tangieren neben den Fragen einer präventiven Entfernung potenzieller Melanomvorläuferläsionen (kongenitale Nävi, sog. atypische oder dysplastische Nävi) v. a. kurative, diagnostische, elektive oder palliative Maßnahmen, auf die in diesem Beitrag aus aktueller Sicht eingegangen werden sollen. …
