Activation of the hypothalamo–pituitary–adrenal (HPA) axis, an integral component of the stress response, results in an increase in plasma cortisol concentrations. In critical illness, a syndrome of relative adrenal insufficiency (RAI) has been proposed, which is thought to be associated with a worse outcome [
