Die Polyposis nasi als immunologische Erkrankung

Zusammenfassung

Die chronische Rhinosinusitis (CRS) mit Polypen gilt als multifaktorielle Erkrankung, eine eindeutige Erklärung wie es zu den Gewebsveränderungen kommt, fehlt noch immer. Die CRS mit Nasenpolypen wird als meist TH2-basierte und eosinophil-dominierte Entzündung beschrieben, bei der verschiedene assoziierte Erkrankungen das Entzündungsmuster verändern können — möglicherweise ein Grund, warum bis jetzt keine einheitliche Therapie entwickelt werden konnte. Künftig könnte sich eine individuell auf den Patienten zugeschnittene, endotypgesteuerte Behandlung als optimal erweisen.

Frühe orale Toleranz statt Sensibilisierung über die Haut

Personen mit atopischer Dermatitis haben begleitend häufig Nahrungsmittelallergien. Die geschädigte Hautbarriere spielt eine entscheidende Rolle bei der Sensibilisierung und erschwert die orale Toleranzentwicklung. Umso wichtiger ist die Regeneration der Hautbarriere.

Schutz der Schleimhautbarriere bei chronischer Rhinosinusitis

Die Gewebsumgestaltung der oberen Atemwege („airway remodeling“) bei allergischem Asthma und chronischer Rhinosinusitis mit nasalen Polypen (CRSwNP) ist nach der neuen Nomenklatur allergischer Erkrankungen der EAACI ein eigenständiger gewebespezifischer Mechanismus.

Immunologische Endotypen beim CRSwNP – neue Nomenklatur

„Die chronische Rhinosinusitis mit Nasenpolypen (CRSwNP) zeigt immunologisch ein sehr heterogenes Bild“, wie Prof. Moritz Gröger, LMU-Klinikum München, beim Deutschen Allergiekongress 2024 dargestellt hat. Der immunologische Endotyp sei hierbei mit dem klinischen Phänotyp assoziiert.
