Zusammenfassung

Jährlich erkranken beinahe eine Million Menschen an einem Magenkarzinom. Die 5‑Jahres-Überlebensrate beträgt in der westlichen Welt nur 20–25 %. Dies ist hauptsächlich dadurch begründet, dass bei mehr als 70 % der Patienten bereits ein lokal fortgeschrittenes und/oder metastasiertes Karzinom diagnostiziert wird. Die komplette Resektion des Primärtumors inklusive D2-Lymphadenektomie ist die einzig unimodale kurative Therapie. Bei resektablen Tumoren erhöht die Kombination mit perioperativer Chemotherapie die 5‑Jahres-Überlebensraten um knapp 15 %. Zunehmende medikamentöse Optionen inklusive neuer zielgerichteter Substanzen ermöglichen in der palliativen Situation eine Lebensverlängerung bei gleichzeitiger Verbesserung von Lebensqualität und Symptomkontrolle. Karzinome des gastroösophagealen Übergangs unterscheiden sich von Magenkarzinomen hinsichtlich ihrer Ätiologie, Diagnostik und teilweise auch in der Therapie; sie bedürfen daher einer separaten Darstellung.
