Zusammenfassung

Die akute Aortendissektion ist eine seltene und aufgrund der Vielfalt der Initialsymptome häufig verkannte Ursache eines thorakalen Notfalls. Richtungweisend können neben thorakalen Schmerzen Symptome der akuten Aortenklappeninsuffizienz, neurologische Ausfälle oder Zeichen einer viszeralen oder peripheren Minderdurchblutung sein. Unbehandelt weist die Erkrankung eine hohe Frühmortalität auf, zumeist infolge einer Aortenruptur oder Perikardtamponade. Ziel präklinischer Maßnahmen ist daher neben einer ausreichenden Analgesie und Anxiolyse (Opioide, Benzodiazepine) die Kreislaufstabilisation auf moderate Herzfrequenz- und Blutdruckwerte (β-Blocker). Bei begründetem Verdacht auf eine Aortendissektion sollte der Transport der Patienten möglichst in ein zur definitiven Versorgung ausgerüstetes Zentrum erfolgen. Findet sich im Rahmen der Diagnostik (Echokardiographie, Computertomographie [CT]) eine Dissektion mit Beteiligung der Aorta ascendens (Typ-A-Dissektion), besteht die Indikation zur notfallmäßigen Operation. Dissektionen der absteigenden Aorta (Typ-B-Dissektionen) können zumeist konservativ behandelt werden.
