12. Die thorakale Aorta

Zusammenfassung

Die Chirurgie der Aorta umfasst ein weites Spektrum an verschiedenen Aortenerkrankungen mit unterschiedlichsten Herausforderungen im Management. Trotz weitgehend standardisierter Operationstechniken bietet fast jeder Patient eine unterschiedliche klinische Charakteristik (Santini et al. 2007), das weitere Vorgehen und das prozedurale Risiko sind oftmals individuell einzuschätzen und abzuwägen. Trotz aller diagnostischen, technischen und (intensiv-)medizinischen Fortschritte ist die chirurgische Behandlung der Typ-A-Aortendissektion weiterhin eine der riskantesten Operationen. Die Behandlungsergebnisse hängen in besonderem Maße von einer besonders raschen Diagnostik und Zuweisung, einer situativ optimalen Wahl der operativen und interventionellen Techniken sowie vom perioperativen Komplikationsmanagement ab.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
