Effekt liposomaler Lokaltherapie auf die Sicca-Symptomatik des primären Sjögren-Syndroms

Einleitung

Das primäre Sjögren-Syndrom (pSS) stellt die häufigste rheumatologische Erkrankung des Kopf-Hals-Bereichs dar. Aufgrund der vielschichtigen und unspezifischen Beschwerden kommen unterschiedliche medizinische Fachdisziplinen mit dieser Erkrankung in Kontakt. Daher ist ein breites Bewusstsein über die Symptomatik des pSS und der symptomatischen Therapieoptionen von Bedeutung. Insbesondere die effektive Wiederherstellung eines Tränen- und Speichelfilms steht zur Linderung von okulären und pharyngealen Beschwerden im Vordergrund. Produkte unterschiedlicher Pflege- und Inhaltsstoffe stehen hierbei zur Verfügung, wobei keine Studien vorliegen, die aufgrund einer schonenden und effektiven Symptomkupierung des pSS zu einer allgemeinen Behandlungsempfehlung führen.

Material und Methoden

Zwischen Oktober 2007 und August 2012 wurden an der Hals-Nasen-Ohren-Klinik und Poliklinik des Klinikums rechts der Isar 73 Patienten mit einem pSS anhand der Kriterien der amerikanisch-europäischen Konsensusgruppe (AECG) diagnostiziert. Vor und nach 2-monatiger Lokaltherapie mit liposomalen Nasen-, Mund- und Augensprays (LipoSaliva®, LipoNasal® und Tears Again®) wurden zusätzlich zu den AECG-Kriterien subjektive Symptome in visuellen Analogskalen erfasst sowie der Geruch- und Geschmacksinn in Identifikationstests überprüft.

Ergebnisse

Die Xerostomie (92 %), Keratoconjunctivitis sicca (KCS; 92 %) und rezidivierende Speicheldrüsenschwellungen (40 %) waren die führenden klinischen Symptome. Es zeigte sich eine durchschnittliche Speichel- und Tränenmenge von 1,89 g bzw. 12,7 mm. Erhöhte SS-A-/B-Antikörper zeigten sich bei 47 bzw. 29 % der Patienten. Histologisch konnte ein durchschnittlicher Fokusscore von 3 nachgewiesen werden. Extraglanduläre Manifestationen wurden bei 17 % der Patienten beobachtet. Demgegenüber zeigten bis zu 70,5 % der Patienten schwerwiegende lokale Komplikationen. Die liposomale Lokaltherapie bewirkte einen signifikanten Rückgang der Sicca-Symptomatik (Xerostomie: p = 0,0001, KCS und Nasentrockenheit: p = 0,004).

Schlussfolgerung

Eine liposomale Lokaltherapie eignet sich zur effektiven Kupierung der Sicca-Symptomatik beim pSS.
