Präoperatives Screening für Risikofaktoren

Zusammenfassung

Der demografische Wandel und die immer besser werdende Gesundheitsversorgung führen zu einem zunehmenden Anteil älterer, vorerkrankter Patienten in der elektiven orthopädischen Chirurgie. Die Einschätzung des perioperativen Risikos ist ein zentraler Bestandteil der Indikationsstellung und Beratung des Patienten. Risikofaktoren wie Herzinsuffizienz, chronisch obstruktive Lungenerkrankungen, periphere arterielle Verschlusskrankheit, Anämie, Malnutrition, Adipositas, Nikotinabusus und ein insuffizient eingestellter Diabetes mellitus sind im elektiv orthopädischen Patientengut verbreitet. Ein frühzeitiges Screening dient nicht nur der individuellen Einschätzung des Risikoprofils, sondern bietet im elektiven Setting auch die Chance, modifizierbare Risikofaktoren gezielt zu behandeln und damit das perioperative Risiko zu senken. Empfehlungen zur Risikostratifizierung und Einstellung modifizierbarer Risikofaktoren haben mittlerweile auch Einzug in die nationale Leitlinie zur Indikationsstellung zur elektiven Hüfttotalendoprothese gefunden.
