Despite advances in technique, patient selection, and outcomes for cytoreductive surgery and hyperthermic intraperitoneal chemoperfusion (CRS-HIPEC), this procedure remains one of the most morbid of all major abdominal operations.
1 In addition, because of the specialized nature of this treatment, the case variety and complexity, as well as the relative low volume compared with other major abdominal procedures, CRS-HIPEC has not lent itself to standardized perioperative protocols such as enhanced recovery after surgery (ERAS).
2 Due to the length of the procedure, the potential for significant blood loss, the magnitude of the surgical insult, and the use of chemotherapy and hyperthermia, concerns about fluid restriction have traditionally outweighed concerns of fluid overload. This article addresses the feasibly, safety, and potential benefits of fluid restriction during CRS-HIPEC.
