Papilläres Schilddrüsenkarzinom in einer medianen Halszyste

Primärtumor oder Metastase?

Zusammenfassung

Vom papillären Schilddrüsenkarzinom ist bekannt, dass es sich häufig erst sekundär durch die lymphogene, zervikale Metastasierung bemerkbar macht. Andererseits ist das Phänomen der malignen Transformation medianer Halszysten beschrieben. So kann in 1% der Fälle histologisch ein maligne entartetes Gewebe nachgewiesen werden. Bei etwa 94% dieser inzidentellen Malignome handelt es sich um dystope Schilddrüsenkarzinome.
Im beschriebenen Fall wurde bei einer 38-jährigen Patientin eine seit 18 Monaten bestehende, monströse, zystische, median-zervikale Raumforderung exstirpiert. Klinisch ähnelte der Befund einer atypischen medianen Halszyste. Histologisch zeigte sich jedoch ein papilläres Schilddrüsenkarzinom. Ob dieser Befund einem primären Karzinom des ektopen Schilddrüsengewebes oder einer zystischen Halslymphknotenmetastase entspricht, soll diskutiert werden.
