Bei Aphasien handelt es sich im engeren Sinne um erworbene Sprachstörungen infolge einer Hirnläsion, etwa durch Schlaganfälle oder eine neurodegenerative Erkrankung. Im klinischen Alltag können Auffälligkeiten der Sprache jedoch auch ein Symptom sehr unterschiedlicher Krankheitsentitäten sein, die sich mit einer Enzephalopathie, zum Beispiel in Form eines Delirs, manifestieren. In Abhängigkeit des Störungsprofils, der Begleitsymptome, des zeitlichen Verlaufs sowie apparativer Diagnostik kann auf den Schädigungsort und die zugrundeliegende Pathologie geschlossen werden. Diese Unterscheidung ist bedeutsam für die Diagnosestellung und Therapie von Sprachstörungen.
