Therapieoption bei Patienten mit analgetikainduzierter Erkrankung der Atemwege

Zusammenfassung

Die spezifische orale Toleranzinduktion (SOTI) mit Acetylsalicylsäure (ASS) hat sich in der Therapie der analgetikainduzierten Erkrankung der Atemwege als zusätzliche Therapieoption bei rezidivierender chronischer Rhinosinusitis mit Nasenpolypen bewährt. Aufgrund des Risikos schwerwiegender Nebenwirkungen wird dringend eine stationäre Durchführung empfohlen. Die Beachtung notwendiger struktureller Voraussetzungen, formaler Bedingungen sowie der Indikation und Kontraindikationen ist auch medikolegal wichtig. Als Dauertherapie werden derzeit 300 (– 500) mg ASS empfohlen. Die Indikation besteht bei persistierenden Symptomen trotz intensiver medikamentöser Therapie und/oder rezidivierender Polyposis nasi, die wiederholt Operationen notwendig macht und/oder bei Patienten, die häufiger Kortison einnehmen müssen, um nasale Symptome oder das Asthma zu kontrollieren. Wenn die ASS-Einnahme von mehr als 48 h unterbrochen wurde, wird die erneute SOTI empfohlen, da mit einem erneuten Auftreten von Intoleranzreaktionen zu rechnen ist.
