Supplementäre Therapie in der perioperativen Phase

Zusammenfassung

Seit der Veröffentlichung zweier Fallberichte, die als die ersten klinischen Nachweise einer iatrogenen Nebenniereninsuffizienz betrachtet werden, gehört es zur allgemein anerkannten Praxis, Patienten unter Glukokortikoidtherapie perioperativ für einen chirurgischen Eingriff mit Kortisol abzudecken. Dabei sind sowohl die Einschlusskriterien der Patienten wie auch das Ausmaß des Substitutionsschemas eher auf empirischer als auf rationaler Basis entworfen worden. Obwohl sich in den letzten 50 Jahren neue Erkenntnisse über die Physiologie des Hypothalamus-Hypophysen-Nebennieren-Systems und die molekularen Wirkmechanismen der assoziierten Substanzen angesammelt haben, sind die sich daraus ergebenden Konsequenzen für die Klinik bisher weitestgehend ignoriert worden. Die klinischen und experimentellen Nachweise stützen jedoch die Ansicht, dass selbst Patienten unter aktueller Glukokortikoidbehandlung für einen chirurgischen Eingriff keine maximalen Stressdosen von Hydrokortison benötigen, die aus heutiger Sicht eher für die Behandlung der Sepsis als reserviert gelten. Unter Berücksichtigung des enormen Wirkspektrums der Glukokortikoide und deren Nebenwirkungen erscheinen eine Revision und Modifikation des historischen Regimes erforderlich.
