Operative Therapie des Endometriumkarzinoms

Zusammenfassung

Die operative Therapie ist die entscheidende Säule in der primären Behandlung des Endometriumkarzinoms. Mit der zunehmenden Inzidenz der Adipositas wird nicht nur die Häufigkeit des Endometriumkarzinoms zunehmen, auch die damit verbundenen operativen Probleme stellen erhöhte Anforderungen dar. Die bisherige operative Standardtherapie per Laparotomie sollte in den Stadien I und II durch die laparoskopischen Verfahren ersetzt werden, da diese bei onkologischer Gleichwertigkeit entscheidende Vorteile für die Patientinnen mit sich bringen. Der Stellenwert der roboterassistierten Chirurgie beim Endometriumkarzinom muss weiter evaluiert werden. Die derzeit am meisten kontrovers diskutierte Fragestellung in der operativen Therapie des Endometriumkarzinoms betrifft den Umfang und die Indikation der Lymphonodektomie in den verschiedenen Tumorstadien. In fortgeschrittenen Stadien sollte eine möglichst komplette Resektion des Tumors erfolgen, um die Effizienz der adjuvanten systemischen und strahlentherapeutischen Maßnahmen zu verbessern.
