Therapie der Belastungsharninkontinenz bei Frau und Mann*

Zusammenfassung

Die Behandlungsmöglichkeiten der Belastungsharninkontienz haben sich in den letzen Jahren stetig entwickelt. Für die Frau wurde die Effektivität der Bandapplikation als ebenbürtig zur klassischen Operation nach Burch belegt. Durch eine kontinuierliche Weiterentwicklung von der retropubischen über die transobturatorische bis hin zu den aktuellen kurzarmigen Bandtechniken konnte die Invasivität kontinuierlich reduziert werden.
Resultierend konnten für die Belastungsharninkontinenz des Mannes Operationstechniken entwickelt werden, welche die geschlechtsspezifischen Unterschiede berücksichtigen. Zunehmend etablieren sich unterschiedliche Bandsysteme in der Therapie der männlichen Belastungsharninkontinenz. Eine weitere Individualisierung im Therapieregime konnte durch die Entwicklung adjustierbarer Systeme erzielt werden, wodurch ineffiziente „bulking agents“ verlassen und die Therapieoption zum artifiziellen Sphinkter ergänzt wurden.
